Quality Improvement (QI) Essentials Toolkit

IHI’s QI Essentials Toolkit includes the tools and templates you need to launch a successful quality improvement project and manage performance improvement. Each of the ten tools can be used with the Model for Improvement, Lean, or Six Sigma, and includes a short description, instructions, an example, and a blank template.

The QI tools include:

  • Cause and Effect Diagram: Also known as the Ishikawa or fishbone diagram, this tool helps you analyze the root causes contributing to an outcome.
  • Failure Modes and Effects Analysis: Also used in Lean management and Six Sigma, FMEA is a systematic, proactive method for identifying potential risks and their impact.
  • Run Charts and Control Charts: These charts help you monitor performance and visualize variation.
  • PDSA Worksheet: Plan-Do-Study-Act rapid-cycle testing helps teams assess whether a change leads to improvement using a methodical learning process.

Download the complete toolkit with all ten tools, or download individual tools as you need them to guide your continuous improvement work.

*NOTE: Before filling out the templates, first save the PDF files to your computer. Then open and use that version of the tool. Otherwise, your changes will not be saved.

ABMS has recently announced that the submission of PA’s is now functional in MOCAM.  As we stated in the July Sponsor Update, there are requirements that must be followed for the NCCPA in order for credit to be issued.  They are as follows:

1.            Review and understand FAQs regarding PA completions

2.            Utilize the marketing template provided by the NCCPA

3.            Utilize the PA Approval Statement for QI Efforts in which PA's will participate for credit

4.            Utilize the PA Attestation Form

5.            Provide PA's with a Confirmation Letter upon completion

6.            Utilize the “PA Completion Notification” form in MOCAM

7.            Utilize the PA Completion table for PA submissions

The NCCPA has been added to the list of ABMS Member Boards in the QI Effort form as a means of indicating that the QI Effort will be available to PA’s.  As of August 28th, 2017, any QI Effort for which PA’s will receive credit must comply with the above requirements.  Any PA that completed a QI Effort on or after May 10th, 2017 may be submitted (using the PA Completion table) for credit through the MOCAM system (it is understood that these completions will not have met all of the requirements listed above).
